Form 4VA For CST Appeal

1. Login using Userid and Password as used during any e-service login such as e-filing of VAT return.
2. Fill up the Online Form 4VA.
3. Click ‘Submit’ button to submit the data entered.
4. Click ‘LOGOUT’ to exit.
5. Acknowledgment giving Form 4VA with entered value will be displayed for taking print.
6. Signed copy of the Acknowledgement to be submitted at Charge office.
